Graduate Certificate in Chronic Disease Management

Program Overview

This eight-month graduate certificate program provides nurses with specialized training related to the prevention and management of chronic diseases. These diseases impairs everyday physical and mental functions and reduce the ability to perform daily activities. Chronic disease management is a specialization that allows nurses to apply their practical knowledge to create patient care programs for chronic conditions. As a graduate of this program, you will be well equipped to treat patients with chronic health conditions including cardiovascular diseases (heart and stroke), cancer, diabetes, arthritis, chronic respiratory diseases and chronic depression. You will also acquire the skills to manage a patient’s chronic condition, plan an individualized intervention plan, educate patients on self- management plans and assess a patient’s condition based on outcome tools.

Work Experience
Field Placement
You will complete 150 hours in a clinical placement at a local acute care hospital, rehabilitation centre or a long-term care facility and apply your skills with the supervision of a clinical supervisor.
